Why Every Visit Starts With A Physical Exam

Most people walk in already convinced they know what’s wrong. A tight shoulder means a torn rotator cuff. Shooting leg pain means a herniated disc. Recurring headaches mean something serious is being missed. Sometimes that’s true. Often, it isn’t, and the only way to actually know is to look.

Why the Exam Comes First

At River of Life Chiropractic, every visit starts the same way regardless of what brought you in: a real conversation about your history, followed by a hands-on exam, before any adjustment happens. We’re checking range of motion, running orthopedic and neurologic tests, and palpating the area in question. That sequence tells us far more than a guess based on where it hurts.

Two people can describe the exact same symptom and have completely different things driving it. A dull ache in the lower back after a long shift on your feet isn’t the same problem as a sharp, radiating pain that starts at the hip and travels down the leg, even though both might get described the same way over the phone. The exam is what tells us the difference, and it’s the reason two patients with the “same” complaint can walk out with completely different care plans.

Why Imaging Comes After, Not Before

Digital X-rays, taken in-house, come after the exam, not before it, and only when the findings actually point to a reason for them. That order matters. Imaging confirms what the exam already suggests, it isn’t a substitute for the exam itself, and it isn’t something we reach for automatically just because someone walks in with pain.

This is also why we don’t do generic treatment plans. A plan built around an actual finding looks different from a plan built around an assumption, and the difference usually shows up in how quickly, and how fully, someone actually improves.

What to Look for in a Provider

Whoever you see, a few things are worth expecting from a first visit, here or anywhere else.

  • A real history. When it started, what makes it better or worse, whether it travels, and what it’s keeping you from doing.
  • A hands-on exam before any adjustment. Range of motion, orthopedic and neurologic testing, and palpation, not a guess based on where it hurts.
  • Imaging only when it adds something. X-rays should follow the exam findings, not replace them.
  • A clear explanation. You should understand what was found, why it matters, and what the plan and cost look like before you start.
  • A willingness to refer. If the exam points somewhere outside chiropractic, you should hear that plainly.

What a Visit Looks Like at River of Life

Your first visit starts with a little paperwork and a sit-down conversation with Dr. Levi Hansen about your history. From there he moves into the physical exam, and takes digital X-rays in-house when the findings call for them. Then he walks you through what he found, using a spine model or your X-rays so you can see it for yourself, and tells you plainly whether he believes he can help.

Care visits follow the same three steps: Prime, guided breathing to help your body settle before anything else happens; Facilitate, a gentle, specific adjustment based on what the exam found; and Integrate, what happens between visits as your body adapts to that input.

When to Go to the ER Instead

Part of the exam’s job is ruling out problems that don’t belong in a chiropractic office at all.

When to call 911 or go to the ER instead

These are reasons to get emergency care first, not to book a visit.

  • Numbness in the groin or inner thighs, or new trouble controlling your bladder or bowels, especially with back or leg pain. These can be signs of cauda equina syndrome, which needs immediate medical attention.
  • Sudden loss of balance, vision changes, a drooping face, arm weakness, slurred speech, or a severe headache with no known cause. These are stroke warning signs. Call 911.
  • A stiff neck with fever and headache, which the CDC lists among common meningitis symptoms.
  • Pain after a fall, accident, or other injury where a broken bone is possible.

What This Means for Your Care Plan

A care plan built on an actual exam looks different for every patient. Someone managing a recent flare-up needs something different than a pregnant patient, an athlete, or someone coming in periodically just to stay ahead of things before anything starts hurting at all. That’s the whole point of starting with an exam instead of starting with an assumption, the plan follows the finding, not the other way around.

If you’ve been putting off getting checked because you’re not sure whether it’s “serious enough” to warrant an appointment, that uncertainty is exactly the kind of question a real exam is built to answer. You don’t have to already know what’s wrong before it’s worth coming in.

I recently experienced a sciatic nerve issue that caused significant discomfort and made everyday activities extremely difficult. Even simple tasks like putting on my socks, shoes, or pants in the morning would take me at least 20 minutes because of the pain and limited mobility. I came here for treatment, and within a month, I was amazed at how much better I felt. The pain that had been interfering with my daily routine was gone, and I was able to get back to my normal activities. I’m very grateful for the care and professionalism I received. I highly recommend this establishment to anyone experiencing similar pain or discomfort. They made a significant difference in my quality of life!

Dr. Hansen actually took X-rays of my spine and went over them with me in detail while explaining my care plan. It was such a different experience from other chiropractors I’ve seen, where treatment began without first getting a clear picture of what was actually going on. Having now experienced both approaches, the difference is astounding. I really appreciate the thoroughness and thought that went into understanding my condition before beginning treatment.

I came in to see Dr. Hansen about 3 months ago. I was having pretty severe neck pain daily. I was struggling with extreme migraines at least 2-3 days a week. After the new patient exam he found underlying issues that were causing the pain and headaches. He made a plan for me and I’m so happy to say that my daily neck pain is gone, I’ve had one migraine since I started. On re exam day I was literally in tears because I had no idea how much those issues were affecting my daily functioning. My overall quality of life has been improved. I am so grateful for the Dr. and this practice! They genuinely care about their patients and take their time to listen and find solutions. 10/10 highly recommend⭐️⭐️⭐️⭐️⭐️

I first started seeing Dr. Hansen because I hurt my lower back at the gym. He got it moving and feeling better within the first visit and helped my neck issues as well. I have much more range of motion in my neck and back and no more pain. He’s personable, friendly and professional. I highly recommend him to everyone. For me chiropractic care has been essential to my wellness routine.
