SI Joint Pain or Sciatica? How to Tell the Difference
Pain in your buttock that runs into your leg gets called sciatica a lot. Sometimes it is. Other times, the source is the sacroiliac (SI) joint, where your lower spine meets the pelvis. The two can feel surprisingly alike, but they come from different places and don’t call for the same care. Here’s how they compare and why an exam is the best way to sort them out.
The Short Answer
Sciatica comes from a nerve. SI joint pain comes from a joint. MedlinePlus describes sciatica as pain, weakness, numbness, or tingling in the leg caused by injury to or pressure on the sciatic nerve, and notes that it’s a symptom of another problem rather than a condition by itself. SI joint pain, according to Mayo Clinic, most often shows up in the buttocks and lower back and can spread into the legs, groin, or thighs.
Because both can send pain down the leg, the location of the pain alone often isn’t enough to tell them apart.
Where Sciatica Usually Hurts
Sciatica usually runs from the low back or buttock down the back of one leg, following the path of the sciatic nerve. When something presses on that nerve, MedlinePlus lists these common patterns:
- Pain on one side, felt as tingling, a dull ache, or burning
- Pain or numbness on the back of the calf or the sole of the foot
- A leg that feels weak, or a foot that catches on the ground when you walk
- Pain that gets worse after standing or sitting, at night, or when you sneeze, cough, or laugh
Common causes include a herniated disc, spinal stenosis, and piriformis syndrome. Because it’s a symptom rather than a diagnosis, sciatica always has a source worth finding.
Where SI Joint Pain Usually Hurts
SI joint pain usually centers just below and inside the bony bump you can feel at the back of the pelvis, according to a StatPearls review from the National Library of Medicine. It can spread into the back or side of the thigh and sometimes below the knee. There’s one SI joint on each side of the pelvis.
Mayo Clinic notes that SI joint pain tends to get worse with prolonged standing or sitting, climbing stairs, running, and sleeping on the painful side. Pregnancy, falls onto the buttock, arthritis, and heavy lifting with a twist are common contributors to SI joint dysfunction.
How They Compare
Here’s a side-by-side look at the usual tendencies:
- Where it starts: sciatica comes from an irritated nerve. SI joint pain comes from the joint between the spine and pelvis.
- Where it goes: sciatica often runs down the back of the leg below the knee, sometimes to the foot. SI joint pain usually stays in the buttock and low back, though it can reach the thigh and occasionally below the knee.
- Nerve signs: numbness, tingling, weakness, or reflex changes point more toward a nerve.
- What flares it: coughing or sneezing often aggravates sciatica from a disc. Stairs, running, and lying on the sore side tend to aggravate the SI joint.
These are tendencies, not rules. Research on SI joint pain notes that SI-related leg pain can follow the same path as nerve-root pain, and that telling them apart from symptoms alone is almost impossible. Some people have both at once.
What to Look for in a Provider
If your buttock or leg pain has lasted more than a couple of weeks, a few things are worth asking about before you book.
- An exam before any adjustment. Neurologic and orthopedic testing, so care is based on what’s found rather than a guess.
- Imaging only when it adds something. X-rays should follow the exam findings, not replace them.
- A plan you can follow at home. Good care includes what to do between visits, not just what happens on the table.
- A willingness to refer. If the exam points to a nerve problem that needs an MRI or a specialist, you should hear that plainly.
- Clear communication. You should understand what care involves, how long it may take, and what it costs before you start.
How an Exam Tells Them Apart
Since the symptoms overlap, the exam does the sorting. It starts with a detailed history: where the pain is, what flares it, and what you’ve already tried. Then come neurologic tests of strength, reflexes, and sensation along the nerve’s path, plus orthopedic tests. MedlinePlus notes that pain when your straight leg is lifted while you lie on your back is one sign of sciatic nerve involvement. StatPearls describes separate provocation tests that stress the SI joint itself, and pain that shows up with several of them points toward the joint.
That’s why every visit starts with an exam. Digital X-rays are taken in our office only when the findings call for them, and if the exam shows significant nerve signs, we’ll refer you for an MRI. If you already have an MRI, keep in mind that bulges and herniations show up often in people with no pain at all, so the report is only part of the picture.
What a Visit Looks Like at River of Life
Care here follows three steps: Prime, guided breathing to help your body settle; Facilitate, a gentle, specific adjustment based on what the exam found; and Integrate, the movement, sleep, and home exercises that happen between visits. Whether the exam points to the SI joint, a nerve, or both, our sciatica and low back pain care is built from those findings.
When Leg Pain Needs Help Right Away
Most buttock and leg pain isn’t an emergency, but some symptoms need medical care first.
When to get medical care right away
These are reasons to see a doctor or go to the ER, not to book a chiropractic visit.
- Numbness in the groin or inner thighs, or new trouble controlling your bladder or bowels. These can be signs of cauda equina syndrome, which needs immediate medical attention.
- Leg weakness that is new or getting worse.
- Back or leg pain with a fever, or after a fall or accident where a broken bone is possible.
- Pain with unexplained weight loss, or pain that won’t ease no matter how you lie.
What to Notice Before Your Visit
A few details make your first exam more useful. Before you come in, notice:
- Exactly where it hurts, and whether it’s one side or both
- Whether the pain goes below the knee
- Any numbness, tingling, or weakness in the leg or foot
- What flares it: sitting, standing, stairs, rolling over in bed, coughing, or sneezing
- What eases it, even a little
Write it down if you can. Those details point toward the source faster than a pain score does.

I recently experienced a sciatic nerve issue that caused significant discomfort and made everyday activities extremely difficult. Even simple tasks like putting on my socks, shoes, or pants in the morning would take me at least 20 minutes because of the pain and limited mobility. I came here for treatment, and within a month, I was amazed at how much better I felt. The pain that had been interfering with my daily routine was gone, and I was able to get back to my normal activities. I’m very grateful for the care and professionalism I received. I highly recommend this establishment to anyone experiencing similar pain or discomfort. They made a significant difference in my quality of life!Posted on Google![]()

Dr. Hansen actually took X-rays of my spine and went over them with me in detail while explaining my care plan. It was such a different experience from other chiropractors I’ve seen, where treatment began without first getting a clear picture of what was actually going on. Having now experienced both approaches, the difference is astounding. I really appreciate the thoroughness and thought that went into understanding my condition before beginning treatment.Posted on Google![]()

I came in to see Dr. Hansen about 3 months ago. I was having pretty severe neck pain daily. I was struggling with extreme migraines at least 2-3 days a week. After the new patient exam he found underlying issues that were causing the pain and headaches. He made a plan for me and I’m so happy to say that my daily neck pain is gone, I’ve had one migraine since I started. On re exam day I was literally in tears because I had no idea how much those issues were affecting my daily functioning. My overall quality of life has been improved. I am so grateful for the Dr. and this practice! They genuinely care about their patients and take their time to listen and find solutions. 10/10 highly recommend⭐️⭐️⭐️⭐️⭐️Posted on Google![]()
