How Do I Know If My Lower Back Pain Is Sciatica?

Quick Summary: Ordinary low back pain usually stays in one spot and gets sharper with certain movements. Sciatica is different. It radiates down your leg because it’s a nerve being irritated, not just a sore muscle. Knowing which one you’re dealing with changes how we approach your care.

Most of the back pain we see is mechanical, the ache from an awkward lift, a long day at a desk, or overdoing it around the house.

Sciatica feels different. We see patients on a weekly basis who come in describing numbness, tingling, or even a burning sensation down their legs, convinced the problem is in their leg. More often than not, the real cause is in the spine, and most people are surprised to hear that.

Both usually trace back to pressure or irritation on the nerves that leave the spine, just at different points. A sore muscle and a compressed nerve are two different problems, even when they feel similar at first. Rather than guess based on where it hurts, we use digital X-rays to get an objective look at your spine, so we know which one we’re actually dealing with before treatment starts.

When Back Pain Needs Emergency Care, Not a Chiropractor

Most sciatica and low back pain is mechanical, exactly what we’re set up to help with. But a few warning signs mean you need a doctor or the ER first:

  • Sudden loss of bowel or bladder control, or numbness in the groin or inner thigh (possible signs of cauda equina syndrome, a rare but serious nerve emergency)
  • Leg weakness that’s getting worse by the hour, or a leg giving out
  • Numbness or trouble walking following a major fall, car accident, or other significant trauma
  • Fever, chills, or unexplained weight loss along with your back pain
  • Chest pain, shortness of breath, or pain that doesn’t match a typical sciatica pattern

How We Know Your Care Is Working

Pain isn’t always a reliable signal on its own. You can have a pain-free day while a nerve is still irritated, or feel sore from yard work while you’re actually improving. So instead of relying on how you feel that day, we track things directly.

Every plan starts with a baseline: your pain levels and your personal goals, recorded on day one. From there, we pay attention to function, not just pain, things like whether you’re sleeping better, moving more easily, or getting back to activities you’d stopped doing. At the end of your plan, we compare new X-rays side by side with where you started, so you can see the change for yourself instead of just taking our word for it.

If your exams and X-rays show real progress, we continue the plan. If they don’t, we adjust the approach instead of just hoping. And if a care plan doesn’t get you the results we expected, we’ll say so and figure out why, rather than continuing the same plan without a reason.

What Happens During a Visit: Prime, Facilitate, Integrate

Every visit follows the same three-step process, in that order, because each step serves a different purpose.

  • Prime: We start with guided breathing to help settle your nervous system before anything else happens. A tense body doesn’t adjust well, so this comes first.
  • Facilitate: This is the adjustment itself, a specific, targeted correction based on what we find in your exam and X-rays, using primarily the Diversified technique, with Activator or Upper Cervical Drop used as needed. Nothing generic.
  • Integrate: This is what happens after you leave, as your body adapts to the adjustment in daily life between visits.

Sciatica pain lives mostly in the nervous system, not just in the discs and muscles, which is why treatment isn’t just about cracking your back and sending you on your way. Your first visit runs a little longer than the ones after it, since we’ll go over your X-rays together and walk through what we’re seeing in plain terms. After that, visits get shorter as we get to know your spine.

Common Sciatica Mistakes That Delay Relief

Sciatica doesn’t always respond to the instincts that work for an ordinary ache, and a few well-intentioned habits can actually slow recovery down instead of speeding it up. A few patterns we see often that end up delaying recovery:

  • Resting too long. A week or two off can ease pain temporarily, but it also weakens the muscles supporting your spine, so pain often returns once normal activity resumes.
  • Stretching or icing only where it hurts, without addressing what’s actually irritating the nerve further up the chain. Leg pain is usually the symptom furthest from its actual source.
  • Switching treatments or approaches too quickly, before giving any one of them enough time to work.
  • Tracking progress only by how you feel day to day, with no objective way to know if you’re actually improving.

Shallow, stressed breathing also keeps your nervous system on guard, which is part of why every adjustment starts with settling it first. Most of these habits come from treating sciatica like a sore muscle, when the actual source is a nerve that needs to be identified, not just soothed. Getting an accurate picture of what’s happening early on is usually what shortens the road to relief, rather than another few weeks of guessing.

How many visits will it take before I notice a difference with my sciatica?

It depends on how significant your exam and X-ray findings are, but most sciatica care plans run on a structured timeline, often around 12 weeks. As you progress, we look at your symptoms, function, and X-ray changes together. If you’re not seeing the progress we expected by a certain point, we’ll say so and reassess the plan rather than just continuing it as-is.

Will I be adjusted on my first visit for sciatica?

Not automatically. Your first visit starts with X-rays and an exam, so we know what we’re actually working with before anything else happens. Once we’ve got that picture, we’ll walk you through what we found and what we recommend from there.

Can yard work or sitting at a desk in College Station make sciatica worse?

Yes. Bending, twisting, and long stretches of sitting all put extra load on an already irritated nerve, and we see it constantly, patients who spent a weekend on yard work or a week hunched over a desk and show up thinking they’ve undone their progress. I came in to see Dr. Hansen about 3 months ago. I was having pretty severe neck pain daily. I was struggling with extreme migraines at least 2-3 days a week. Dr. Hansen is an amazing chiropractor. I wasn’t sure anyone would be able to help me with my neck. It has been hurting for months. I thought I slept wrong, but wasn’t sure why it wasn’t going away.
