Quick Summary: A herniated disc and a pinched nerve are related but not the same thing. A disc can herniate without touching a nerve, and a nerve can be pinched by something other than a disc, like a restricted joint or inflamed tissue. Telling them apart matters because it changes what we actually treat.
The two terms get used interchangeably, but they describe different problems. A herniated disc means the soft cushion between two spinal bones has torn or bulged. A pinched nerve means something, disc material, bone, or swollen tissue, is pressing on a nerve as it exits the spine. You can have one without the other, and it is common to see patients who assume they have a disc problem when the exam points to something else entirely, like a joint that stopped moving well.
Rather than treating a description of symptoms, we treat what the exam and imaging actually show. Digital X-rays taken in our College Station office let us see what is going on structurally instead of guessing based on where the pain travels.
An image cannot tell us what your leg feels like at 4 a.m. That is why we start by listening to your story, when the pain began, what makes it worse, and how far it travels, before we ever touch you.
From there we move into a hands-on exam: range of motion, orthopedic tests, neurologic testing, and palpation. Pain that stops at the buttock behaves differently than pain that runs past the knee, and the exam is what tells us which one we are actually dealing with. Reflexes, strength, and sensation testing help confirm whether a specific nerve root is involved.
Digital X-rays support the physical exam. They never replace it. We use them to confirm what the hands-on findings already suggest, not as the first step in figuring out what is wrong.
Care follows the same three-step process for every patient: Prime, Facilitate, Integrate. With a herniated disc or an irritated nerve, that order matters even more than usual.
The goal is reducing pressure on the nerve root and restoring movement to the joints guarding around it, not just chasing the pain wherever it happens to be that day.
Most disc and nerve problems respond well to conservative care. A few signs mean you need a doctor or the ER first, not a chiropractic visit:
This cluster of symptoms is rare, but it can point to cauda equina syndrome, which is time-sensitive. If any of these apply to you, get medical evaluation and imaging before any adjustment. Once that is ruled out, most disc and nerve irritation is exactly what we are built to help with.
Pain is not always a reliable measure of what is actually happening with a nerve. You can feel better before the underlying irritation has fully settled, or feel sore from normal activity while still making real progress. That is why we do not rely on how you feel alone.
We recheck the same findings we started with, reflexes, strength, sensation, and range of motion, at set points through your care rather than just asking how today feels. If a finding has not moved yet, we say so directly and adjust the plan, instead of continuing on hope. Many patients notice gradual improvement over several weeks, though sudden, severe cases often take longer to resolve than pain that built up slowly.

Not usually. We start with a full physical exam and digital X-rays in our office, since those guide the plan of care in most cases. MRI becomes necessary only if your exam shows red flag signs, like true weakness or an unclear neurologic picture. Many people with disc changes on MRI have zero pain, so scans alone do not tell the whole story.
It can help both, depending on which nerve is irritated and what is causing it. Once the exam shows which structure is involved, care focuses on reducing pressure on that nerve root and restoring movement to the joints guarding around it. Results vary by person, but many patients notice less pain as inflammation and irritation settle down.
Bring any old X-rays, MRI reports, or notes from a previous doctor, even if you do not understand them yet. We will walk through what they show alongside your own physical exam findings. Old scans without context can be confusing, so we explain what is relevant to your current symptoms and what is not, right there during your visit.
It depends on how long the nerve has been irritated and how severe the compression is, but many patients notice gradual improvement over several weeks of care. Sudden, severe cases from a fall or crash often take longer than pain that built up slowly. We recheck your reflexes, strength, and range of motion regularly to confirm the plan is working, not just guessing.

I came in to see Dr. Hansen about 3 months ago. I was having pretty severe neck pain daily. I was struggling with extreme migraines at least 2-3 days a week. After the new patient exam he found underlying issues that were causing the pain and headaches. Dr. Hansen is an amazing chiropractor. I wasn’t sure anyone would be able to help me with my neck. It has been hurting for months. I thought I slept wrong, but wasn’t sure why it wasn’t going away.
