Quick Summary: Soft tissue therapy can help tight muscles, knotted fascia, and other areas of movement restriction, including low back pain, neck and shoulder tightness, and tension related to stress. It complements chiropractic care well, especially since tight or restricted muscles can be a sign the nervous system is still stuck in a guarded state.
When we start working on an area and the muscles are tight, resisting release, that usually means there’s a pattern within the nervous system the tissue is holding onto. It’s not just a case of finding a knot and rubbing it away. Here’s what that guarded tightness tends to look like:
When we see sciatica-type pain that goes down the leg, the muscles around the hips and low back are usually very tight as well, the body’s way of protecting an inflamed nerve. Easing that protective reaction through soft tissue work allows for more free movement in the area. One thing we’ve noticed in our College Station patients is how much tension collects in the lower back and upper shoulders given how much desk work and long commutes many of them make.
Most folks can be reluctant to address aches. Tightness or pain in your neck or between your shoulder blades will likely subside on its own, but if it happens daily, that’s worth paying attention to. A few signs it’s time to stop waiting it out:
Don’t wait until it gets worse. That kind of daily tightness is your body trying to tell you something, and it’s a sign self-care alone probably isn’t enough anymore. If you’re experiencing pain, come see us now instead of later.
How long has it lasted, and where does it show up? A mild ache in one spot that’s already fading after a few days looks very different from something spreading, or something with an identifiable cause like yard work or a long drive. Self-care is usually enough when:
Self-care can be helpful up to a certain point. If the tightness keeps coming back, or if you also have pain traveling down your arm or leg, self-care has been exceeded. At that point, it would be beneficial to have someone check it out.
Tissue can get tight for a number of reasons, compensating for a nearby joint not moving correctly, or your nervous system asking a muscle to brace and protect an area. That’s why we never guess when assessing something, we want to figure out why it’s tight before we start working on it.
Prime, Facilitate, Integrate. The guided diaphragmatic breathing during Prime quiets a revved-up nervous system, which matters because tight bodies don’t adjust well. Facilitate is the adjustment itself, targeted and gentle, primarily Diversified Technique with Activator or Upper Cervical Drop as needed. Lastly, the Integrate step is what happens after you leave, easing back into movement so your body adapts to the change over the next day or two as you move normally again.
Soft tissue work isn’t a replacement for adjustments, it enhances them. If we adjust a joint while the muscle around it is still guarded, the joint is more likely to slip right back out of place. Working out that guarding first improves the odds the adjustment actually holds.
We build soft tissue work into preparing the body for adjustment, though depending on what your exam finds, that might mean working on tissue before your adjustment, after it, or both, not a fixed add-on tacked onto the end.
Prime starts with diaphragmatic breathing to relax and settle your nervous system before anyone lays a hand on you. Tight tissue and a revved-up nervous system tend to go hand in hand, and easing that guarding is what lets Dr. Hansen adjust your spine effectively. Facilitate is the targeted, gentle adjustment, primarily Diversified Technique, with soft tissue work often performed alongside it since the two complement each other well. Our College Station patients tend to hold a lot of guarding in their necks, shoulders, and low back, often from sitting at a desk or a long commute.
Integration is the stage many offices skip. It gives your body time to actually integrate the treatment instead of rushing you out the door, and tissue that arrived gripping your spine often releases its grip during this stage.
You likely just need rest if mild soreness fades within 3-4 days and improves with stretching or a hot shower. If tightness keeps returning weekly, or you feel numbness or pain shooting down your arm or leg, self-care isn’t enough anymore. That’s when a clinical check makes sense instead of guessing at home stretches that aren’t working.
Soft tissue therapy treats tight muscles, knotted fascia, and movement restrictions, especially in the low back, neck, and shoulders. It also helps tension headaches caused by tight suboccipital muscles at the base of your skull. Many College Station patients come in with sciatica-type leg pain, where tight hip and low back muscles are protecting an inflamed nerve. Releasing that tension helps restore normal movement.
Desk work and long commutes, like the drive up and down Texas Avenue, build tension in your neck and upper trapezius muscles. Your nervous system often braces that area in response to daily stress, not just poor posture. This is why foam rolling or a heating pad alone doesn’t fix it. The tightness is a coping pattern your body has learned, and it usually needs hands-on assessment to release fully.
It depends on your specific issue, so there’s no one-size-fits-all order. Some tight knots respond well without an adjustment first, while other aches need soft tissue work before an adjustment helps. Dr. Levi Hansen assesses each patient during the visit to figure out the right sequence for you. It’s a clinical decision, not guesswork, based on what your body shows us that day.
Expect a clinical assessment first, before any hands-on work begins. Dr. Levi Hansen checks why an area is tight, whether it’s compensating for a nearby joint or your nervous system bracing for protection. This matters because some muscles resist release if the nervous system is still guarded. Skipping this step and just rubbing out a knot rarely gives lasting results, so the assessment comes first.
If a knot returns within a week even after treatment, that’s a sign self-care and one-time work have been exceeded. Recurring tightness in the same spot usually points to a nervous system pattern, not just a muscle problem. Common triggers we see in College Station include long stretches of desk sitting, sitting in Texas A&M game-day traffic, or a weekend spent moving boxes or doing yard work.

I recently experienced a sciatic nerve issue that caused significant discomfort and made everyday activities extremely difficult. Even simple tasks like putting on my socks, shoes, or pants in the morning would take me at least 20 minutes because of the pain and limited mobility. I came here for treatment, and within a month, I was amazed at how much better I felt. The pain that had been interfering with my daily routine was gone, and I was able to get back to my normal activities. I’m very grateful for the care and professionalism I received. I highly recommend this establishment to anyone experiencing similar pain or discomfort. They made a significant difference in my quality of life!Posted on Google![]()

Dr. Hansen actually took X-rays of my spine and went over them with me in detail while explaining my care plan. It was such a different experience from other chiropractors I’ve seen, where treatment began without first getting a clear picture of what was actually going on. Having now experienced both approaches, the difference is astounding. I really appreciate the thoroughness and thought that went into understanding my condition before beginning treatment.Posted on Google![]()

I came in to see Dr. Hansen about 3 months ago. I was having pretty severe neck pain daily. I was struggling with extreme migraines at least 2-3 days a week. After the new patient exam he found underlying issues that were causing the pain and headaches. He made a plan for me and I’m so happy to say that my daily neck pain is gone, I’ve had one migraine since I started. On re exam day I was literally in tears because I had no idea how much those issues were affecting my daily functioning. My overall quality of life has been improved. I am so grateful for the Dr. and this practice! They genuinely care about their patients and take their time to listen and find solutions. 10/10 highly recommend⭐️⭐️⭐️⭐️⭐️Posted on Google![]()

Dr. Hansen is an amazing chiropractor. I wasn’t sure anyone would be able to help me with my neck. It has been hurting for months. I thought I slept wrong, but wasn’t sure why it wasn’t going away. Dr. Hansen explained that pain is often the last thing to show up and that my neck had been needing care for longer than I’ve had pain. Then he adjusted me and explained what he was doing throughout the visit. It took 4 visits for the pain to go away and my sleep, energy, focus, sinuses, and headaches have all improved. I will be continuing care as I know it will ensure I maintain my health! I will be sending my friends and families into to see him.Verified by TrustindexTrustindex verified badge is the Universal Symbol of Trust.
